| You will also need to read and fill out the Health Information Privacy Protection Act Form and send it in along with the camper application and physical examination form. |

| We must have this information. Funding or a letter advising of funding from these sources must be in prior to camp attendance, otherwise, camper will not be accepted upon arrival. |

| Camper applications are DUE back to Wonderland Camp by May 1. Remember, first applicants are assured of their choice of weeks. After this date it will depend upon availability. Please contact the camp office at 573-392-1000 or email us at info@wonderlandcamp.org for availability. |
